RFP - How to Apply for a Grant
Susan G. Komen for the Cure, Columbus Affiliate is pleased to announce the availability of grant funding. Last spring we funded 34 local programs ifrom the $1.4 million raised at our 2007 Race. This year, due to our continued success in 2008, the Columbus Affiliate of Susan G. Komen for the Cure is seeking qualified grant proposals for funds to be distributed in 2009.
Deadline for completion of applications is November 14, 2008 and awards will be announced in February 2009. Monies for the first half of the grant cycle will be distributed April 1, 2009 upon completion of a fully executed contract. Please note the following:
- Applicants must provide evidence of non-profit status.
- Applications may be made to support either new or established programs.
- Applicants must provide one original and twenty (20) copies of the grant application to the Susan G. Komen Breast Cancer Foundation, Columbus Affiliate, Grants Committee, Komen Columbus, 855 Grandview Ave, Suite 250 , Columbus, Ohio 43215
- Deadline for receipt of proposals is November 14, 2008.
To receive a copy of our 2009-2010 grant application please join us for our Grant Workshop Conference Call. The workshop provides potential grant applicants with instruction, resources and tools to improve the quality of their grant funding proposals to Komen Columbus. This workshop is a way to educate potential grant applicants about the Komen Foundation and extend the opportunity to apply for a Komen grant to as many community organizations as possible.
We have scheduled two dates:

Qualifications: Applications are accepted from you as non-profit institutions: U.S. citizenship or residency is not required. Applications must be submitted in English. Applicant organizations must be located and/or providing services within any of the following Ohio counties which make up our service area: Athens, Delaware, Fairfield, Fayette, Franklin, Gallia, Hocking, Jackson, Lawrence, Licking, Madison, Marion, Meigs, Morgan, Muskingum, Perry, Pickaway, Pike, Ross, Scioto, Union, Vinton, and Washington (service area).
Emphasis: The purpose of this program is to address the breast health and/or breast cancer screening, treatment, diagnosis, educational and emotional needs of the Komen Columbus Affiliate service area. Through our community needs assessment, we have identified the following needs:
Franklin County
• Increase screening rate of all women over 40 through increasing awareness, improving access and reducing barriers.
• Maintain a high rate of mammography of African American women while increasing the mammography rate of white women and women eligible for Medicare/Medicaid or without any insurance.
• Provide culturally sensitive programming and services to our diverse ethnic/minority populations.
• Provide vouchers for breast health-related prescriptions, childcare, wigs, prosthesis, and transportation to breast health medical appointments.
• Provide assistance to women undergoing diagnostic procedures not covered by insurance or other funding sources.
• Provide programs that address the needs of survivors, including education, treatment for side effects, exercise, nutrition, and psychosocial support.
Contiguous / Appalachian Counties
• Increase screening rate of all women over 40 through increasing awareness, improving access and reducing barriers with a special emphasis on women eligible for Medicare/Medicaid or without insurance.
• Provide assistance to women undergoing diagnostic procedures not covered by insurance or other funding sources.
• Provide vouchers for breast health-related prescriptions, childcare, wigs, prosthesis, and transportation to breast health medical appointment.
• Provide programs that address the needs of survivors including education, treatment for long term side effects, exercise, nutrition, and psychosocial support.
• Breast Health programs from counties that do not currently have breast health initiatives.
Special consideration will be given to those proposals which address any of the above unique needs of the Central Ohio area.
